Field Care Coordinator / Account manager

This role will be full time 40 hours permanent contract

36 hour shifts within 1-1 care and support

4 hours within the office coordinating

Job duties:

  • Communication/liaise with management

  • 12 hour shifts to assist/provide personal care; Provide Nutrition and hydration support; Support with medication needs; Assist in domestic duties; Comply with all policies and procedures;

  • To report back any concerns relating to the health of the service user;

  • To work together to constantly improve services

  • Coordinate and manage the package of care

Weekly rota basis - in advance to know your shift patterns- through IT System Care Planner;
Permanent contract- 40 hours contract or 20 hrs with overtime available if wanted;
